This Clarification Text has been prepared by the Republic of Turkey Ministry of Health (Ministry), which is the data controller within the scope of the Law on the Protection of Personal Data No. 6698 ('KVK Law'), in accordance with Article 10 titled 'Obligation of the Data Controller to Disclose', in order to inform the users of the Centralized Doctor Appointment System (CDAS) about the personal data of the users. The headquarters address of the Ministry, which has the title of data controller pursuant to the PDP Law, is "Bilkent Campus, Üniversiteler Mah. Dumlupınar Bulvarı 6001. Cad. No:9 Çankaya/Ankara 06800".
The data controller for your personal data processed in this system is the Republic of Turkey Ministry of Health.
In this system, your personal data below can be processed for the following purposes.
- Identity data: In CDAS, your T.R. Identification Number, name, surname, father's name, mother's name, place of birth, date of birth, age and gender information of you and your children under the age of 18, which you can see under the tab "I am authorized", can be processed in order to verify your identity and to determine the appointments you can make.
- Contact data: In CDAS, the landline phone number, mobile phone number or e-mail address information of you and, if any, your children under the age of 18, which you can see under the tab 'I am authorized', can be processed for the purposes of contacting you via SMS, live operator, robotic call information methods in order to maintain appointment services during the membership phase, and if you do not have an CDAS Mobile or web membership, to forward your appointment information you have received by the call center, to inform and remind you when there is any appointment change. You have the right to determine your own preference through your web or mobile memberships for our method of contacting you via the phone information we have received.
- Location data: The location information of your location in CDAS, which you and your children under the age of 18, which you can see in the tab ‘’I am authorized’’, if any, is used only instantaneously and not recorded so that you can easily reach the address of the hospital where you have made an appointment with any navigation tool, as you can determine with the options "continuously" or "only when using the application" from the location information preferences of the CDAS Mobile application you have downloaded to your smart mobile device. You can determine when it should be used by choosing. If your location services are turned off, your location information is not processed by CDAS.
- Health data: The date and time of the examination appointment that you and your children under the age of 18 have made or canceled, the hospital/institution/clinic/doctor where the appointment was made, featured patient code, patient appointment number, disability status, vaccination date and time, vaccination appointment number that you have made or canceled, which you can see in CDAS, can be processed for the purpose of conducting business continuity activities, conducting vaccination appointment processes, conducting citizen notification processes (SMS, Push, e-mail, etc.). Your information on whether you have the right to vaccination, how many doses of vaccine you can make an appointment for, whether you have the right to receive vaccination at home, and which type of vaccine you have the right to receive is used instantly and is not recorded.
- Audio and visual data: Your voice recording personal data is received in transactions made through the call center and may be processed in order to carry out the transaction security of the persons receiving and providing appointment services.
- Transaction security data: User name and password information in CDAS can be processed in order to carry out business continuity activities and to carry out citizen membership transactions.
Your personal data in the Centralized Doctor Appointment System is not shared with third parties in any way, without prejudice to the exemption cases in Article 8 and the first paragraph of Article 28 of the KVK Law.
Your personal data is obtained by fully automated means (through this system) and the legal basis for the processing of your personal data is that it is mandatory for the data controller to fulfill its legal obligation pursuant to subparagraph (ç) of the second paragraph of Article 5 of the KVK Law and (f) data processing is mandatory for the legitimate interests of the data controller, provided that it does not harm the fundamental rights and freedoms of the person concerned, and in accordance with the third paragraph of Article 6; for the purposes of protection of public health, preventive medicine, medical diagnosis, medical diagnosis, treatment and care services, planning and management of health services and financing, by persons or authorized persons or authorized institutions and organizations under the obligation of confidentiality.
Users of the Centralized Doctor Appointment System may exercise their rights regulated in Article 11 of the PDP Law by applying to the Ministry within the framework of Article 13 of the PDP Law and the provisions of the Communiqué on the Procedures and Principles of Application to the Data Controller.
Written applications to be made pursuant to Article 13 of the KVK Law shall be submitted to "Republic of Turkey Ministry of Health, Universities Mahallesi, 6001. Cadde, No:9, Çankaya, Ankara" address; applications to be made by Registered Electronic Mail (KEP) should be sent to "sb@hs01.kep.tr" address.
